Scheduling Your First Appointment


Tips for your appointment: 


  • Call or Email us to provide your contact information. Please have insurance information at hand . 

 

  • For transfer of records from your child's previous provider please complete a Medical Records Release. Records are required before we can see your child so that we may provide necessary continuity of care. To expedite the process, new patients are welcome to download our forms online before their visits.

 

  • Please note medical offices have up to 30 days to complete your transfer request, so it may take some time before we are able to schedule your child's appointment. If your child is sick or needs to be seen urgently, please call our office to speak with one of our patient service representatives.


At the 
wellness visit, the provider will get an idea of every-day life for your child and address any questions or concerns you have. They will assess your child's growth and development as well as school performance for older children. The provider will also perform a complete physical exam of your child. 

For teenagers, you may be asked to step outside the room to allow the provider to build a trusting relationship with your child and to help your teen start taking part in their own health care.
