Login Information for CHADIS & Tips

In an effort to better serve our patients, we have partnered with CHADIS, Child Health and Development Interactive System.  CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office.

• After you complete the questionnaire, CHADIS results come directly to your doctor.
• The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.
• We use CHADIS for all of your child's checkups, at all ages, and additionally use it for behavioral and mental health evaluations.

 

The CHADIS website will automatically assign the questionnaires that we use based on the age of your child(ren). Once you have completed the registration, the questionnaires will take about 10-20 minutes, depending on your child’s age. Please complete these no sooner than 2 weeks before your appointment.

Returning Users: 

• If your child is 11 or older, your child will be the one completing the questionnaire, and should do so using their own login.
• Please go to CHADIS within 2 weeks of your child’s physical exam to complete the assigned questionnaires for the visit.
• If you are unable to complete CHADIS before your visit, please arrive 15 minutes before your appointment time, in order to complete it in the office.

New Users:

2. Enter our practice invitation code: 4343857776
3. Complete the registration.
4. Your email address is an easy to remember username.
5. Please keep track of your password, as you will be using the CHADIS system prior to every well visit.
6. Enter your child’s information.
7. Select “Take Questionnaires.”
8. Select the doctor that you are seeing and the appointment type – this will automatically populate the questionnaires based on age.
9. Complete the questionnaires.
10. Click “Send It, I’m Done” at the end of each questionnaire.

 

If you do not have a chance to complete the questionnaire on the CHADIS website, please arrive 15 minutes prior to your appointment time so you can fill them out in the office.
